The admitting/patient access office is located in the lobby of the main entrance of Rappahannock General Hospital. You may register between the hours of 7 a.m. and 4 p.m., Monday through Friday. If for some reason you are unable to register during normal office hours, you may go the emergency department and register there. (Please keep in mind that if the emergency department is busy, you may have to wait.)
When you come to register, please bring the following with you:
- All of your current insurance cards
- Photo ID
- The physician order form for the test or procedure you need. Make sure your doctor’s signature and the space marked “Principle DX or Reason for Procedure” are completed. If not complete, you need to notify your doctor’s office.
You can download the Consent for Treatment form; bringing this completed will save time in the registration process. You may also download the Financial Policy Agreement to review prior to registering.
If you are registering a minor who is not your child, please make sure that you bring the completed “Consent for Treatment of a Minor” form.
Once you have registered, our staff will direct you to your next destination.
